Blepharoplasty: A Comprehensive Guide to Eyelid Surgery

Blepharoplasty - better known as eyelid surgery - is a surgical procedure in which excess skin on the eyelids is removed. This can be performed on both the upper and lower eyelids. As part of the procedure, other issues such as excess fat pads in the eyelid area can also be addressed. The aim of the surgery is to refresh the eye area, making it look less tired and heavy. In addition, the removal of excess skin on the upper eyelids often results in improved peripheral vision.

What is an eyelid lift?

An eyelid lift is a surgical procedure on the eyelids that aims to create a more youthful appearance of the eye area. This can be performed on both the upper and lower eyelids. Furthermore, excess fat deposits that are responsible for swelling of the eyelids can also be removed. The combination of removing excess skin and fat deposits provides a fresher and younger look. The procedure is performed on an outpatient basis under local anesthesia.

When should one undergo an eyelid lift?

Most patients opt for an eyelid lift for aesthetic reasons, particularly due to a tired appearance and puffy eyes. However, if there is a relevant limitation of vision caused by excess skin on the upper eyelids, this constitutes a medical indication for the procedure.
The ideal time to perform the surgery cannot be determined by a general rule; many factors play a role, such as individual genetic predisposition or premature skin aging due to excessive sun exposure.

What are the preparation and aftercare like?

First, the indication for the procedure is verified during a consultation with a trusted surgeon. An ophthalmological examination with a visual field test can substantiate the medical indication for the surgery. It is particularly important to consider the eye in its entirety. For example, excess skin on the upper eyelids can also be caused by a significantly drooped eyebrow. During the consultation, potential risks and possible complications are discussed, as well as the aftercare.

How long does the recovery take and what does it involve?

Primary wound healing is completed after one week. At this point, the sutures are also removed; social ability is generally restored by then. In the first two days after the procedure, there is moderate swelling and redness around the eyes. Less frequently, small bruises may also become visible. During this healing phase, we recommend cooling measures. In addition, physically strenuous activities should be avoided, as these increase blood pressure, which can cause secondary bleeding.
Once the stitches have been removed, it is recommended to massage the scars daily with a panthenol-containing cream. This supports scar maturation and is a good measure for preventing hardened scars. A final result is visible no earlier than three months after the operation.

Is there an ideal patient for eyelid surgery?

Ideally, we are dealing with a healthy patient in good general condition who has a visible and understandable problem in the sense of a relevant excess of skin on the eyelids and/or bothersome fat pads. The patient's age is not a relevant factor. The indication for an eyelid lift can consistently be present as early as age 35.
